Warshak Parent Questionnaire

AN ORGANIZED FORMAT FOR DEVELOPMENTAL AND FAMILY HISTORIES

A good evaluation requires lots of information from parents, usually more information than you have time to obtain. From a mental health and legal perspective, it is important that you fulfill your professional responsibility to assess all necessary aspects of a child's psychological status.

That is why, for the past eighteen years, your colleagues throughout the country have turned to the WPQ to improve the quality of their evaluations and document their work. It is a particularly valuable addition to social studies, custody evaluations, consultations, and mediation because it provides a cost-effective means of eliciting comprehensive data with no extra investment of your time.

  • A complete, organized developmental history
  • Symptom checklists keyed to DSM-IV
  • Parents' knowledge of their children's problems and personality strengths
  • Comparison of relative involvement of mother and father in direct contact with child
  • Family history of psychological problems
  • Ratings of child's self-esteem and peer relations
  • Stressful and traumatic life events history (Axis IV)
  • Data on family constellation, including stepparents and stepsiblings
  • School adjustment, child's temperament, medical history
  • Information on possible parental alienation

Previously, the WPQ was available only as a parent-completed booklet. Although this format is still available, the new online version of the WPQ has several distinct advantages:

  • The questionnaire is administered online wherever internet access is available. Booklets cannot be misplaced, out of stock, or lost in the mail.
  • Questions are tailored: only items germane to a particular child are presented.
  • Drop-down menus ensure that questions are answered appropriately.
  • A comprehensive narrative report is emailed to you the instant the parent completes the WPQ.
  • The questionnaire report is presented in a natural, flowing text style.
  • The report is easy to "cut and paste" into your own evaluation report.

"It's the greatest thing since sliced bread for developmental history. It's made my work easier!"
